Overview

Improvements in the health of the population require interventions beyond the clinical interfaces. Population Health Management examines the outcomes desired for the health of the population, identifies the determinants of health and their influences and constraints, and then intentionally implements policies and programmes to address the rate-limiting factors.

This executive education programme establishes the concepts and objectives underlying Population Health Management. It will provide the principles and insights for sound design in Population Health Management (and its critical differences from clinical care, disease management, and integrated care), the framework to identify challenges and develop steps required for its successful implementation, and the skillsets for the monitoring and evaluation of complex projects. Examples of Population Health Management initiatives around the world are examined for their paradigms and principles of design and contextualised for Singapore.

Certification

An electronic Certificate of Competence will be awarded to participants who pass all assessments and meet a minimum of 75% attendance.

Learning Objectives

At the end of the programme, the student will be able to:
1.     Articulate the foundational concepts and paradigms of Population Health
2.     Analyse and establish sound objectives for Population Health initiatives
3.     Analyse and construct sound dynamics for Population Health systems
4.     Apply systems thinking to manage complexity and constraints in Population Health
5.     Integrate and synthesise evidence for design, implementation, monitoring and evaluation of policy and programmes
6.     Apply appropriate levers to create the biggest impact to the health of the population
7.     Appreciate the principles in designing an appropriate monitoring and evaluation framework for Population Health
8.     Architect a sound systems approach for improving the Health of the Population

Who Should Attend?

Mid-level management in the design and implementation of policy and programmes as well as for senior management with oversight for Population Health.

Pre-requisite

A minimum requirement of a bachelor’s degree and/or two years’ work experience.

Course Instructors

Adjunct Professor Lee Chien Earn

Prof Lee is currently the Deputy Group CEO of SingHealth overseeing the Regional Health System which seeks to enable the population in Eastern Singapore to Keep Well, Get Well and Live Well. His prior appointments included leadership positions in the Ministry of Health in Health Services, Healthcare Performance, Health Regulation and Health Services Finance in the Ministry of Health, Singapore.

Jason Yap
Associate Professor Jason Yap

Assoc Prof Jason Yap is a public health physician with vast experience in the public and private healthcare sectors with diverse responsibilities covering public policy, informatics, marketing and education. He now serves as the Vice Dean (Practice), Associate Professor (Practice) and Director (Public Health Translation) in the NUS Saw Swee Hock School of Public Health.

Besides his key academic interests in Health Professionals Education, Healthcare Management and Integrated Care, he has taught on a wide range of topics including research and evaluation, strategic planning, systems thinking, healthcare marketing and medical tourism/travel, information security, and medical ethics and professionalism.

Ng Yeuk Fan
Adjunct Associate Professor Ng Yeuk Fan

Dr Ng is a Senior Consultant Public Health Physician experienced in health systems and services planning and evaluation, healthcare corporate transformation, organisation development and change management.

He is currently the Director & Head of Corporate Development at Yishun Health, a member of the National Healthcare Group. As the Director, he is responsible for value-based care & services planning, health system & services evaluation, insights & analytics, corporate planning, and organization development. Ongoing focus areas include the development and deployment of a Unified Care Model for the Yishun Zone Regional Health System in NHG, development of a health system and health services performance measurement framework, value-based transformation of lifelong care and episodic care services, development of person-centered needs assessment tools, implementation of needs-based population segmentation tool, development of population outcomes and financial risk prediction models, deployment of population health survey for the North of Singapore, development of a unified data model for population health systems, and development of organisation development methods to accelerate health system transformation.

ANG Ian Yi Han
Assistant Professor Ian Ang 

Dr Ang is an Assistant Professor (Practice) at the Saw Swee Hock School of Public Health, National University of Singapore (NUS). He spent my formative years in Singapore, then spent 12 years in New York City for school and work, before moving back to Singapore. His research interests include exploring how the built environment can help nudge communities towards improved health and evaluating community-based health interventions and healthcare services.

SkillsFuture Singapore (SSG) Funding Requirements

This is a SkillsFuture Singapore (SSG) approved course. As part of SSG funding requirements, participants must attain at least 75% attendance rate and pass all assessment(s) to successfully receive the certificate.

Participants of the SSG-approved course should adhere to the fee payment timeline of 120 days from the course start date. Individuals or Organisations will be liable to pay the full course fees in the event that learners fail to meet the attendance/ assessment requirements and the other terms and conditions for SSG funding eligibility.

Cancellation

  • Cancellation is permissible if conveyed to the course administrator in writing via email before 22 March 2024.

  • The School reserves the right to cancel the course and fully refund the fees in the event of unforeseen circumstances.
