Overview
This 3-day executive programme equips healthcare and social care professionals with the practical frameworks, tools, and mindsets needed to design, implement, and evaluate integrated care initiatives. Through hands-on exercises, real-world case studies, systems thinking methods, and structured change frameworks, participants learn how to analyse current systems, identify integration opportunities, craft value-driven care models, and lead sustainable transformation across organizational boundaries.
Learning Objectives
By the end of the course, participants will:
- Understand the foundational principles, vocabulary, and value proposition of integrated care.
- Apply systems thinking approaches to analyse healthcare services fragmentation and identify leverage points in their own service environments.
- Use population segmentation, need analysis, and risk stratification models & other tools/methods to design integrated care models.
- Apply practical change and implementation frameworks to plan, sequence, and scale integrated care initiatives.
- Evaluate integrated care design and enablers using structured analytics and evidence-based frameworks.
Who Should Attend?
Healthcare and social care leaders, programme managers, and system planners involved in designing, implementing, or evaluating integrated care initiatives.
Certification
An electronic Certificate of Participation will be awarded to participants who meet a minimum requirement of 75% attendance rate.
Pre-requisites
A minimum requirement of a bachelor’s degree and/or two years’ work experience.
